If your patient is experiencing a dental emergency please call 206.501.5221 and leave a message Monday-Friday 9am-5pm. If there is a Saturday emergency please call 206.232.0333 and press 1.
Examples of dental emergencies include fractured teeth when the injury is less than 48 hours old, fractures of the maxilla or mandible, or severe oral pain which prevents eating and drinking.
For routine referrals, please fill in the form and have your client call 206.501.5221 to set up a consultation with Dr. Crocker. Consultations are on rotating afternoons Monday-Thursday. The basic consult fee must be prepaid at the time of scheduling to reserve a spot.
Routine surgery is conducted Monday-Thursday. Emergency surgery may be scheduled based on medical need.
